The KARAT School of Learning is always striving to keep our parents and students informed of  changes and updates in schedules and new information as it hits.   Having the completed forms for each student, helps the KARAT School of Learning run more efficiently.  All of KARAT's forms are available online for your convenience.  Simply fill out the forms and either emial back to us or print and send in via mail.

1.Student Application - To be filled out in full by the potential student only.
2.Student Assessment Questionnaire - Filled out by studentfor Admissions Board interview.
3.Parent Application - To be filled out by the potential parent or parents. (coming soon) 
4.Emergency Contact and Medical Form - Medical information and release form.
5.Transportation Permission Form - For transporting your child to and from Field Experiences.
6.Field Experience Permission Form - Permission for your child to attend Field Experiences.
